- Ongoing comprehensive case management and wrap-around services coordination to designated Members by collaborating with other organizations and resources
- Comprehensive assessment of Member’s needs and, together with Member, develop a comprehensive emergency and longer-term care plan which includes appropriate goals and resources
- Develop problem-solving relationships with Members, and the designated KI Angel Volunteer assigned to each Member
- Assist in expanding KI’s resource network to meet Member needs
- Treat Members with dignity, compassion, and understanding
- Record client-related information timely on Slack or other designated systems
- Monitor Member plans and progress for periodic re-assessment
- Maintain strict confidentiality of Member information and records consistent with KI protocols
- Attend weekly and monthly case management team meetings
- Provide input as part if continual organizational process improvement program
- Two to five years of related human services experience to include comprehensive Social Work case management services, including assessment, service planning, coordination, and documentation
- Master’s degree in Social Work (MSW) preferred, Bachelor’s in Social Work required.

- Flexible hours: Average 24-32 hours/week part-time
