Social Work Case Manager

Job Description:

Kindness Initiative (KI) serves people in the San Diego Jewish community who are facing the consequences of living on or below the poverty line by providing a unique customized case management approach to their emergency needs and a path to self-sustainability.

KI is seeking a qualified Social Work Case Manger to provide professional assessment, advice and ongoing support to Members (clients), including: conducting intake calls and biopsychosocial assessments with future KI Members; developing, coordinating, and monitoring individualized service plans; identifying resources based on Members’ needs; advocating for Members; following up with Members and their assigned volunteer angels; documenting all phases of Member interactions; evaluating each plan’s effectiveness; and other tasks that may arise.

Responsibilities include: 

  • Ongoing comprehensive case management and wrap-around services coordination to designated Members by collaborating with other organizations and resources
  • Comprehensive assessment of Member’s needs and, together with Member, develop a comprehensive emergency and longer-term care plan which includes appropriate goals and resources
  • Develop problem-solving relationships with Members, and the designated KI Angel Volunteer assigned to each Member
  • Assist in expanding KI’s resource network to meet Member needs
  • Treat Members with dignity, compassion, and understanding
  • Record client-related information timely on Slack or other designated systems
  • Monitor Member plans and progress for periodic re-assessment
  • Maintain strict confidentiality of Member information and records consistent with KI protocols
  • Attend weekly and monthly case management team meetings
  • Provide input as part if continual organizational process improvement program

Qualifications include: 

  • Two to five years of related human services experience to include comprehensive Social Work case management services, including assessment, service planning, coordination, and documentation
  • Master’s degree in Social Work (MSW) preferred, Bachelor’s in Social Work required.
  • Personal drive to alleviate poverty in the San Diego Jewish community
  • Kind, compassionate, and empathetic personality
  • Demonstrated knowledge and network of community resources addressing varied Member needs
  • Experience in client advocacy and skilled clinical intervention
  • Proven development and maintenance of effective working relationships with clients, team members, and supervisors
  • Sensitivity to Jewish culture and familiarity with the San Diego Jewish community preferred
  • Kind, compassionate, and empathetic individual with the ability to:

– maintain boundaries with clients

– use independent judgment and initiative

– be organized and complete and maintain accurate, orderly and timely records

– discuss sensitive matters in a non-judgmental manner and to respect confidentiality

– work effectively with people of diverse demographics

– promote KI program services and network with partner organization

– collaborate in a team environment

  • Effective time management skills
  • Basic computer skills

 

Position Structure:

  • Reports to: Case Management Program Director
  • Flexible hours: Average 24-32 hours/week part-time
  • Work setting: Remote work flexibility where appropriate
  • Salary: Paid on an hourly basis and commensurate with experience

Kindness Initiative

The mission of Kindness Initiative is to alleviate poverty in the San Diego Jewish Community by meeting the basic physical and psychological needs of vulnerable Jewish Households with dignity, kindness, empathy and Jewish values. Kindness initiative refers to those it serves as “Members.” Members needs are met through a unique, customized, concierge, efficient, and effective client service model through an in-depth professional wrap-around case management program that identifies key issues of Members and provides detailed, supportive avenues for overcoming their challenges, including professional case management, a broad resource network of organizational collaborations, an extensive trained volunteer corps, and innovative customized resource and Member solutions. Kindness Initiative has assisted approximately 400 individuals and expects to increase client intakes by as much as 50% in 2023.

Kindness Initiative has a separate division, kindnessG’MACH, which accepts donations of a broad array of household goods and distributes them to Jewish households in need. Both divisions service similar clienteles and there are meaningful cross referral opportunities and synergies.
